生性活泼的贤秀（申世京 饰）是一个被妈妈深爱着的17岁的高中女生，妈妈尹熙（度智媛 饰）是一名外科整容医师，深得人心。
贤秀对妈妈的工作——面部整形毫无关心，但是贤秀的朋友却对整形手术特别的关心，其中贤秀的知己秀静（俞多仁 饰）决心放假期间在贤秀妈妈那里接受整形手术。
在手和脚被捆绑的情况下躺在手术台上的秀静，在手术要开始瞬间，突然眼睛里出现了不知道为何物的黑色物体，这时候她感到前所未有的恐惧，秀静在全力挣扎的时候被麻醉药物的作用渐渐地入睡。
手术后，秀静变的有点让人认不出来，同时在和贤秀、在熙（安雅英 饰）慧苑（全小敏 饰）一起所学的美术学院里很自信的做石膏雕刻模特儿。
秀静虽然受到朋友的妒忌，但是自从整形手术后一直被自己的脸即将被刀撕破的幻想所捆饶。
并紧接着遭遇自己的脸被刀撕破的同时悲惨的死去，贤秀看到这个恐怖的场景后非常惊愕，紧接着在尹熙那里接受过整形手术的在熙和慧苑相互割破对方的脸相继悲惨的死去，看到这样的景象贤秀母女陷入了恐惧中。
目睹朋友惨死的贤秀在最要好的朋友成恩（安桂莲 饰）那里得到安慰，在精神状况稍微得到镇定时，偶然间进了从小时候就开始被禁止出入的地底下的仓库。
那里铺满了整形前后的少女照片，她还发现了一个小孩子恐怖的大头像，受惊后的她大叫，究竟妈妈隐瞒了什么
